If you do it in the compiler (static stack caching), you typically
have a canonical stack state, and force the stack representation into
this state at the start of a definition and at other places where you
would otherwise get problems. Gforth returns to the canonical state
(one stack item in a register) at every basic block boundary (branch,
branch target). For straight-line code, the compiler always knows in
which state the previous VM instruction ended and can compile the
appropriate version of the current VM instruction. For more on that,
read [ertl&gregg05].
If you do it in the VM interpreter (dynamic stack caching), every VM
instruction knows at which state it ends, and jumps to the version of
the next VM instruction for that state (similar to what you describe
for a microcoded CPU). In that case the compiler does not need to be
aware of stack caching. For more on that, read [ertl95].

abstract = "An interpreter can spend a significant part of its
execution time on arguments of virtual machine
instructions. This paper explores two methods to
reduce this overhead for virtual stack machines by
caching top-of-stack values in (real machine)
registers. The {\em dynamic method} is based on
having, for every possible state of the cache, one
specialized version of the whole interpreter; the
execution of an instruction usually changes the
state of the cache and the next instruction is
executed in the version corresponding to the new
state. In the {\em static method} a state machine
that keeps track of the cache state is added to the
compiler. Common instructions exist in specialized
versions for several states, but it is not necessary
to have a version of every instruction for every
cache state. Stack manipulation instructions are
optimized away."

abstract = {Stack caching speeds Forth up by keeping stack items
in registers, reducing the number of memory accesses
for stack items. This paper describes our work on
extending Gforth's stack caching implementation to
support more than one register in the canonical
state, and presents timing results for the resulting
Forth system. For single-representation stack
caches, keeping just one stack item in registers is
usually best, and provides speedups up to a factor
of 2.84 over the straight-forward stack
representation. For stack caches with multiple stack
representations, using the one-register
representation as canonical representation is
usually optimal, resulting in an overall speedup of
up to a factor of 3.80 (and up to a factor of 1.53
over single-representation stack caching).}
