We need to make a distinction between a user-mode-context taking care of an exception it itself generated in its own context; and a user-mode-context taking an exception on behalf of another user-mode-context.
Signal handlers are representative of the former desire, and indeed, it takes WAY too much overhead to "get to" a user-mode signal handler. {Part of the problem is that the user-mode signal handler is not restricted in any way and can longjump(anywhere) in the user-mode-context. AND the general assumption that a context can only have a single exception pending at any instant in time.}
However, entire subsystems COULD be written as user-mode-contexts whereby one context "calls" another context to perform a function that it itself does not have access or access rights to perform. The OS and VM don't need to get into the game except for setting up the tables to connect the dots. However, almost no current machines are architected in this manner.

In effect, there are a dozen or so instruction performed in a highly priveledged state that are simply doing what the HW should have done on behalf of the context running at the instant of the interrupt.

I submit that this is NOT the INETRRUPT HANDLER, but microcode that just happens to exist as executable instructions. The interrupt handler is the code that reads the device registers, figures out what caused the interrupt, and does something about it. The sanitary setup and sanitary teardown are code sequences that attempt to maintain the illusion to the user-mode-context that nothing happened between instructions. I submit that it is this setup and teardown code that should the HW.
