IME with business computing a company wouldn't just leave a programmer 
to get on with coding. When I worked for a bank we had a design-and-test 
review after a programmer had designed the program (to review the design 
and the test plan) and two other separate processes to test the new 
program before it went into production. So programmers were not left to 
take responsibility for everything themselves. The team became jointly 
responsible for what was written.
