Yep. The fan on the Radeon HD 4870 is attached to a heat sink with
thermal paste between the heat sink and the GPU. Replacing it requires
you to remove the heat sink and thermal paste and replace it with new
thermal paste (and the new fan). The RAM chips are cooled with thermal
pads connected to the card chassis rather than thermal grease to the
heat sink. Here's what the poor thing looked like when I first took it
apart:

You can see that the thermal paste had hardened up a bit, and probably
wasn't doing as good of a job transferring heat from the silicon to the
sink. I bought this Mac Pro back in 2008, and while I do give the
computer a good dusting once or twice a year, I had never taken the
graphics card apart in all that time. So the dust buildup inside of the
graphics card was substantial. Needless to say it runs a lot cooler and
a lot quieter now that I've given it a good cleaning, new thermal paste,
and a new fan. : )
