all it takes is a paper clip into the pinhole on the sim slot.

get the proper size sim to begin with.

it's often free online.

also, stores will often give you one if you activate service, but some
stores try to charge money no matter what.

you'd have to go out of your way to screw it up.

get the appropriate sim (micro or nano, depending on the ipad), put it
in the ipad, go to settings and follow the steps to sign up. 

on rare occasion that won't work and you might need to call the carrier
or go to the carrier's store to get it to work.

voip won't work too well on edge but chances are high that you aren't
going to be stuck on edge.

t-mobile has refarmed most of their network so the chances are very
high you'll have at least 3g.

new york city definitely has refarmed t-mobile.
