It has EVERYTHING to do with data consistency.  You don't want a 
database to point to a file which no longer exists, for instance.  Nor 
do you want a file which is no longer pointed to by the database (and 
therefore unused).

Sure there is.  There is a direct relationship between a user and his 
avatar on a forum, for instance.  Also a direct relationship between a 
picture of a product and its description on a shopping site.

In fact, most images ARE related to something - very few images are 
completely stand-alone.
