Every time I upgrade Calibre (which is about every time I use it), I
get a message saying:

The operation can't be completed because some items had to be skipped.
For each item, choose File > Get Info, make sure "Locked" is
deselected, and then check the Sharing & Permissions section. When you
are sure the items are unlocked and not designated as Read Only or No
Access, try again.

this is after I authenticate with my administrator name and password
and say to replace it.

I haven't figured out how to find which items these are, so I end up
deleting the original copy and trying again.

This doesn't happen with other application upgrades.

Happens to me with FireFox and Thunderbird.
I've learned to delete the old one first.
The alternate method I found was far more work.

This happens when I use a program like MacUpdate.  Caliber itself 
downloads a dmg file which has always opened and installed fine for me.

I should clarify my early message.  FireFox and Thunderbird updates 
don't work for me, possibly because of this issue.  So I download their 
DMG, and if I try to drag the app to where I want it, I get the message
Howard complained about.  Unless I first delete the old version.

Doesn't happen with Opera or other items delivered via DMG.

Ah!  That has happened to me as well.  I don't think it has anything 
specifically to do with the browser.  I am reasonably sure the dmg is 
the same regardless of how it is downloaded.  I have no idea why this 
happens with certain programs but like you I just delete the old version 
before installing the new.

BTW, I don't think I have ever had this problem with Calibre.  I have 
had problems downloading Calibre but not installing it.

I'm not sure quite what you mean.  It obviously does have something to 
do with the .app  My guess is that at least one file in there has an
ACL forbidding replacement.

If you are talking about the app you are trying to install, I have no 
idea why you are having a problem but I have as well.  Like you, my 
solution was to delete the old version before installing the new.  
Frankly, I never really tried to figure out what the problem might be.  
Deleting was so much easier than worrying about it.

If you are talking about how the DMG is downloaded, it shouldn't matter.  
The only problem I have had here is a download occasionally gets screwed 
up, but it seems to be independent of what I am using to do the download 
and running it again usually works okay.
