That's certainly a simple approach, and probably better than a fixed
calling convention, but leaves quite a bit of potential for
improvement: A simple definition calling first A and then B would have
to adjust the stack mapping between the calls.

An alternative, possibly better approach would be to use something
like the coagulating register allocator: Start with one (ideally the
most-executed) word, then propagate its register allocation to
adjacent (in the call graph) words (ideally following frequently
executed edges first); due to cycles in the graph you will have to
adjust the stack mapping at some edges, but ideally these would be
infrequently-executed ones.

However, the coagulating register allocator never caught on, probably
because graph colouring and (alternatively) linear scan are more
effective.  However, it's not clear how to apply these techniques
interprocedurally.

These optimizations are not exclusive, so a compiler can do them both.
Although admittedly a good interprocedural register allocator will
reduce the optimization benefit from inlining and vice versa.

Do you have numbers for that?  I.e., how many of the dynamically
executed calls are late-bound method calls?
