IANAD; you'll have to ask a doctor.

Typically private insurance will cover you for
whatever the doctor recommends. I'll presume
there are clauses relating to not covering for
wacky "experimental" treatments

Not everything in this world has a $ sign attached.

There is the concept of "doing the right thing for
other people/society in general", plus the self-respect
of knowing that you have put the patient's interests
first. But that is speculation on my part; IANAD.

A,l examples of money trumping (ho ho) what is
in patients' best interest.

IANAD. From patients' PoV the question is unimportant.

That may be one reason why US care is so expensive:
lots of unproductive paper pushing admin overheads.

For dedicated professionals, money is usually a
"hygene factor" in Maslow's terms. If you have
enough to be happy, having 10% more doesn't
make you 10% happier.

Agreed; that's why the "go NHS" recommendation is
so powerful.

Geographically yes, but culturally large cities
here are /very/ cosmopolitan. Out in the boondocks
less so, of course.
