The important fact:
From the payer/patient/provider's point of view, this is a CLEAR WIN!
I.e., you WANT to adopt such treatment plans -- despite their cost -- as
they are COST EFFECTIVE (*if* you assume there is a value to the Pt's life
and quality thereof!)

Imagine a similarly expensive treatment for something more widespread
(e.g., hypertension, diabetes, etc.) to offset other costs that would
likely be incurred by those populations. When do you step back and
start questioning addressing the *root* of the problem, instead of
the symptoms? When do you start giving away hypodermics and condoms
instead of expensive antiretroviral drugs?

Or, do you impose price controls on the vendors? Perhaps, force them
to shift their profit making to other markets?

Part of Obamacare was the push to standardize and make electronically
accessible the records of Pts. E.g., if I want to see a different *dentist*
(not addressed by Obamacare), I have to get my previous/current dentist to
FAX the records (often at my cost) to the new one. A dentist still using film
Xrays is at a disadvantage vs. one using digital Xrays, etc.

[I don't think our medical records are in a "national" database whereby
they could be accessed by a GP in another state if I presented, there.
But, I'm sure they could be accessed with far less hassle than they
could have, just a few years earlier. Likewise, I can walk away from
any visit with a LEGIBLE record of my Dx and care just by asking!]

I can walk into an Urgent Care facility to get care without an
appointment. But, it is much more costly than seeing my own PCP.
(OTOH, much LESS costly than the ER!).

"Middlemen". There are innumerable opportunities for this sort of
"value added" service in all aspects of life. And, as people become
more insular in their connections with those around them, I suspect
a growing need (you haven't ASKED your "friends" about their experiences
with Hospice, Doctor X, Hospital Y, etc.)

Of course! But, there is a *cost* to that "relationship". It means
"you" need/want to see provider X and not "equivalent provider Y".
That affects provider X's scheduling/availability which can manifest
as delays in when you *can* see him/her.

It also depends on the peoples involved and their attitudes towards
that interaction. My PCP spends exhorbitant amounts of time with me
because he enjoys teaching (educating) and finds a "willing student"
in me. I *push* him, intellectually, instead of just settling for
"take two and call me in the morning".

Finding ANOTHER provider who will be willing to engage me in that way
will be a crap shoot (my PCP is ready to retire). How will that "new"
relationship affect my attitude regarding the quality of the care I
then receive? How will the "efficiency" change? (How long will it
take for NewDoctor to realize that the information that I volunteer
is *probably* relevant to the Dx and not just casual anecdotes?)

But there isn't a wide variety of equipment needed to practice dentistry.
You don't need a lab, MRI, XRAY, CAT/PET, full surgery, on staff
anesthesiologist, cardiologist, orthopaedic man, etc. AND the staff
to maintain all that kit ("Come back next week; we can't do your FMX
this week cuz our XRAY machine is down")

I broke a bone in my youth. Within hours of presenting at the ER, I was
in surgery.

Is the decision to treat SOLELY that of the MD? Or, is he operating under
guidelines imposed by your health care system? I.e., does he ever have to
justify (to the gummit) his decision to aggressively (expensively) treat
the 92 year-old's Ca?

Here, if your policy covers a particular treatment, your age and other
health issues aren't a factor in whether or not your provider will be
reimbursed for your care.

E.g., my neighbor's wife was a "consequence" (victim?) of a medical
screwup. Her doctor suggested (to her husband) that she be allowed
to die as the quality of her life would be "poor" (how does one determine
the quality of someone ELSE's life?). Yet, the husband decided to
let her live. So, monies were spent keeping her alive (10 years?)
despite the fact that she was essentially a lump of flesh (unable to
eat, defecate, sit up, etc.)

What if the Pt is competent and just doesn't WANT to "let go"?
My CHF friend KNEW he was circling the drain -- yet, couldn't
bring himself to opt for hospice care and the (expedited?)
exit that it provided. His wife and kids had no say in the matter.
Nor did the hospital at which he would present each time he'd
find himself in respiratory distress.

The hospital staff would tell his wife "don't bring him back".
But, there's no way to ENFORCE this; *if* he reappears, they
will "welcome" him back and go through the same routine, again.

Conversely, my esophageal Ca friend refused the esophagectomy
that could have prolonged his life a bit more (beyond the
2 years that the chemo/radiation had given him). Despite his
family's desire to keep him around a bit longer, there was
nothing THEY could do to force the treatment plan on him.

> Of course there are separate hospices for terminally ill patients,

One has to *elect* to go into Hospice, here. I.e., many folks
will, instead, end up dying in a hospital bed (ER, ICU, regular
room) because they didn't opt for the Hospice option.

Again, the Pt has to essentially (effectively) say, "I know that
I am dying. That it is only a matter of time. And, that I
don't want to delay the inevitable -- by any sort of artificial
means (including resuscitation, etc.). Keep me comfortable
until that time..."

And, a certain group of politicians who don't want providers
discussing these options with their patients! (at the same
time, grumbling about how expensive it is to keep these folks
alive -- often on the gummit dole/medicare)

Folks seem terrified of The Inevitable and hope that by
not acknowledging it, it will magically go away! :>

SWMBO's mom was offered a kidney transplant. Then, dialysis (since
she refused the transplant). Despite the fact that she was well
into her 90's.

How much "life" are you entitled to? And at what (who's!) expense?

I haven't met anyone who disliked the CARE they received under Obamacare.
Their complaints were related to the *costs* and the idea that it was
"mandated" (I suspect that if you allowed them to opt out FOR LIFE
they'd be reluctant to exercise *that* option! :> )

Trumpcare -- which is what we have as of 1/20/2017 -- will leave folks
complaining about their cost *and* care. Something the Republicans
are only starting to realize...

Yet another issue that Obamacare attempted to address -- pushing for
coverage for *preventive* care. Of course, folks not needing those
tests (really??? how do you know that you *don't* need them?)
complain because their premiums have increased -- to cover the
tests administered to "others".

There is apparently an elevated risk of folks in my age group
to have "silent" hepatitis so the CDC recommended a one-time
test for everyone in that age group (no other risk factors).
I wonder how many *actually* got tested? For those who
didn't get tested, what cost will The Healthcare System incur
for their care when they become symptomatic?

How many folks actually KNOW they are "clinically obese"?
Vs. how many who just think they've "put on a few pounds"?

<grin>

Test for occult blood. You can buy these OTC as well as have
a doctor "prescribe" the test for you.

Colonoscopy is recommended, here, at 50. And, every 10 years thereafter
if "clean"; 5, otherwise. After 76, "routine" screenings are contraindicated
unless the Pt has a predisposition to colorectal Ca. After 85, nothing.
Of course, if you are 86 and want to "pay cash" and can find someone
willing to assume the *risk* of performing it, you won't be turned away!

Another aspect of Obamacare was to move towards *science* based medicine.
I.e., proving/quantifying the benefits of particular treatment strategies
instead of "winging it". And, trying to mandate coverage for things "known"
to yield positive results instead of covering more dubious procedures
(a friend with MS had all the Hg fillings removed from his mouth in the hope
that *that* was a contributing factor to his illness)

IMO, you can take two approaches to healthcare:
- whatever YOU can afford given your PERSONAL resources
- whatever SOCIETY can afford, given some constraints placed on the resources
it wants to dedicate to that

We seem to have no problem throwing a few trillion dollars away in
ammunition...
