  This thread has been dormant for a while, until Steve Hayes
resurrected it today.

  That made me re-read your OP and made me think that what you seem to
be asking for is actually some kind of 'sync from phone to Windows'
functionality. I.e. your "skip-duplicate" is actually a sync type
operation.

  Having said that, you may want to investigate again and now search for
sync-type apps or Android or Windows.

  One I know is MyPhoneExplorer [1], which has an Android and Windows
component and which can work over USB (and WiFi and Bluetooth).

  MPE has several sync-type functions, one is even called 'Sync photos'!

  So if you haven't already, have a look at MyPhoneExplorer.
