No, in normal face-to-face conversation we use just one name. Your 
family don't call you "Malcolm McLean" (I trust!), but just "Malcolm". 
Surnames are added only when ambiguity needs to be avoided (such as in 
this newsgroup). Middle names are a further way to disambiguate in cases 
where that, too, is necessary. And sometimes even that is not sufficient 
(as in, say, the HMRC database).

That's because one isn't enough here (we have several Richards, for 
example). But two happens to be enough. If it weren't enough (if, for 
example, another David Brown showed up), we'd need some way to tell them 
apart. If neither were happy to be called Dave, we'd hope for a middle 
initial, at least.

The "Rule of Two" is a fiction that fails to take into account the very 
real and very common situations where you need more than two parts to an 
identifier.
