As I understand it the process is in "D" state that means
"uninterruptible sleep" that is usually because waiting for I/O.

I do not know about FreeBSD. When unsure what processes are stuck
behind I/O one can utter on Linux something like:
watch "(ps -eo stat,pid,comm|awk '(NR==1)||(\$1~/D/){print}')"
That will give information about processes in "D" state.

There are no silver bullets. Few kilobyte files are likely
best to read and write "atomically" as whole file. With bigger
files we need to pick asynchronous or synchronous. Trying to mix
those with same file will add complications, no gains. Choice
what is more optimal depends on how expensive processing is
done during I/O. Synchronous communication seems to be easier
for programmers to reason about so pick that as default.
