I was studying some disk r/w code and discovered the author made a
mistake in the error handling.  That bug would only have shown itself
had a disk read error occurred.  The moral of the story is that bugs
lurk in code that is rarely exercised or hard to test.

I suggest bugs tend to be reported (in-house or from clients) rather
than found through formal testing.  I worked in service.  No-one tested
everything because it was impractical for a host of reasons.  The same
applies to software.  Unless a client is prepared to pay for formal testing
(which may not turn up all bugs), product testing will be limited to the
extent the manufacturer thinks is necessary to get it to market.
