The meaning to the compiler is exactly the same - but the meaning to
someone reading (or writing) code is completely different.  Many warning
messages that compilers can give are for code whose meaning in C is
perfectly clear, well-defined and unambiguous, but which is wrong or at
least confusing for the programmer.  For example, compilers (with
appropriate flags enabled) could warn on "if (x = 2) ...", since the
programmer probably meant to write "if (x == 2) ...".  I would put a
warning on such mismatches of parameter names in the same category.

I can understand that the committee did not want to make the names
meaningful outside of the definitions.  I don't think that was a good
decision (I would have preferred mismatches to require a diagnostic at
least), but I believe I can understand why they did it.  However, there
is nothing to stop a compiler adding non-default warning options to
enhance checking in code, to help programmers write safer and more
consistent code.  People who /want/ to have mismatches, or omit
parameter names, can continue to do so - that is their standards-given
right.  And people who want to enforce consistency and check for
accidental mistakes could do so using the warning flags.

(Obviously the place to ask for such a feature is the gcc bugzilla
wishlists, but that does not mean that it cannot be discussed here.  It
turns out that there is an old enhancement request already in gcc
bugzilla, which was closed as "won't fix" - with arguments that I
strongly disagree with.
