A given C implementation defines specific sizes for the various types.
Some compilers might have options to specify different sizes, but then
the compiler invoked with and without such an option would effectively
be two different implementations.
I don't know what you mean by "programs whose meaning would be
unambiguous".

Here's what the standard says: N1570 6.2.5p9:
The range of nonnegative values of a signed integer type is a
subrange of the corresponding unsigned integer type, and the
representation of the same value in each type is the same.
with a footnote:
The same representation and alignment requirements are meant to
imply interchangeability as arguments to functions, return values
from functions, and members of unions.
This applies only to corresponding signed and unsigned types, such as
int and unsigned int; it says nothing about, int vs. long, for example.
So `printf("%x\n", 42)` is "meant" to print 42, even though "%x" expects
and unsigned int argument and 42 is of type int.
`printf("%d\n", 42L)` has undefined behavior *even if int and long
happen to have the same representation*. (Of course an implementation
is free to make it work in the expected manner.)

I don't believe that's correct. The C90 standard discusses pointer
conversions in section 6.2.2.3; it doesn't even mention converting a
pointer of one type to a pointer of another type (other than to and from
void*). Apparently the behavior of converting an int* to a long* was
undefined in C90. Furthermore, C90 6.3 says:
An object shall have its stored value accessed only by an
lvalue that has one of the following types:
-- the declared type of the object,
-- a qualified version of the declared type of the object,
-- a type that is the signed or unsigned type corresponding to the
declared type of the object,
-- a type that is the signed or unsigned type corresponding to a
qualified version of the declared type of the object,
-- an aggregate or union type that includes one of the
aforementioned types among its members (including, recursively,
a member of a subaggregate or contained union), or
-- a character type.
The use of "shall" means that an access via any other type has
undefined behavior. The wording is a bit different in C11 (in
particular it uses the phrase "effective type"), but the general idea
is the same.
This:
int n = 42;
long l = *(long*)&n;
has undefined behavior in C89/C90, in C99, and in C11, regardless of the
representations of int and long.
