How old is the monitor?

If it's relatively new, it could be a manufacturing flaw.

If it's old, it's probably failing.  If you get more
and more dead pixels, I'd say it's definitely failing.  Nothing lasts
forever.  But have a tech check it out anyway.  And start saving for a
new monitor.
