Das eigentliche Problem bei den medizinischen Daten ist nicht, daß sie 
heißbegehrt sind. Sind sie nicht, im Gegenteil. Sondern daß es für 
diejenigen, die sie von Berufs wegen kennen sollten, eine Qual ist sich 
durchzuarbeiten und sie kennenzulernen. Die Hausärzte lesen  nicht die 
Briefe der Fachärzte mit deren Diagnosen, kam diese Woche wieder raus.

AOK zum Hausarzt-Facharzt-Verhältnis
"Dies deutet auf Probleme im Informationsaustausch hin“
Bei der Kommunikation zwischen Haus- und Fachärzten liegt einiges im 
Argen – so lässt sich die Kernaussage des Gesundheitsreports der AOK 
Rheinland/Hamburg auf den Punkt bringen: Die Hausärzte berücksichtigten 
zu selten die Diagnosen der Fachärzte. Dies zeigten Abrechnungsdaten.

Die Kasse untersuchte anhand von Abrechnungsdaten insbesondere, ob 
fachärztliche Befunde und Therapieänderungen in der Behandlung durch 
Hausärztinnen und -ärzte Berücksichtigung finden. „Die Auswertungen 
zeigen, dass dies in vielen Fällen nicht passiert. Insbesondere die 
Behandlung von Krankheiten, die neu durch Fachärztinnen und -ärzte 
diagnostiziert wurden, wird mehrheitlich nicht durch Hausärztinnen und 
-ärzte fortgeführt“, bemängelt der Bericht.

Die Diagnose einer chronischen Lungenerkrankung werde nur in jedem 
dritten, einer Herzinsuffizienz in jedem vierten und einer chronischen 
Nierenkrankheit nur in jedem fünften Fall durch Hausärztinnen und -ärzte 
aufgegriffen. „Insbesondere bei älteren Menschen und Pflegebedürftigen 
zeigen sich Probleme“, heißt es weiter.

Fachärztliche Änderungen bei der medikamentösen Therapie würden bei 
Herzerkrankungen, Typ-2- Diabetes und der chronischen Nierenkrankheit 
zumeist durch Hausärztinnen und -ärzte übernommen. Bei chronischen 
Lungenerkrankungen werde die geänderte Therapie hingegen in weniger als 
der Hälfte der Fälle fortgeführt.

Eine fehlende hausärztliche Weiterbehandlung müsse zwar nicht in jedem 
Fall problematisch sein. Liege aus Sicht der Hausärztinnen und -ärzte 
trotz Kenntnis der Erkrankung kein Therapiebedarf vor, sei eine 
Behandlung auch nicht erforderlich. „Insgesamt ist das Ausmaß der 
hausärztlichen Weiterbehandlung aber zu niedrig. Dies deutet auf 
Probleme im Informationsaustausch hin“, urteilt die Kasse.

In Deutschland komme es zu häufig zu Informationsverlusten aufgrund 
einer mangelnden Kommunikation zwischen den Leistungserbringenden im 
Gesundheitswesen. Internationale Vergleichsstudien zeigten, dass 
Deutschland Schlusslicht in Bezug auf den Informationsaustausch und die 
digitale Vernetzung im Gesundheitswesen sei.

AOK kritisch zur ePA: "Diese Zeit fehlt im Praxisalltag"

Die ePA sieht die Kasse in der derzeitigen Form allerdings noch nicht 
als großen Hoffnungsträger: „Die elektronische Patientenakte (ePA) wird 
zwar aktuell eingeführt. Doch momentan ist sie noch weit davon entfernt, 
Ärztinnen und Ärzten einen schnellen und kompakten Überblick über 
behandlungsrelevante Informationen zu bieten. Es wird eine Vielzahl von 
Informationen in die ePA eingestellt. Sofern sich Ärztinnen und Ärzte 
die Zeit nehmen und konkrete Informationen suchen, werden sie diese in 
der Regel auch finden.“

Es erscheine hingegen unwahrscheinlich, dass Ärzte die unterschiedlichen 
Informationen der ePA durchforsteten, um einen Gesamtüberblick über den 
Gesundheitszustand und die Behandlung ihrer Patientinnen und Patienten 
zu gewinnen. „Diese Zeit fehlt im Praxisalltag, in dem Ärztinnen und 
Ärzte es kaum schaffen, Arztbriefe gründlich zu lesen“, weiß die AOK. 
Nötig sei daher künftig eine kompakten Übersicht, über 
Behandlungsdiagnosen und Arzneimittelverordnungen, die einzeln 
angewählt, entsprechend behandelt und unkompliziert übernommen werden 
könnten.
