I have the same problem.
Setting "dm_mod.use_blk_mq=0 scsi_mod.use_blk_mq=0" works well.

$ cat /sys/bus/scsi/devices/0\:0\:0\:0/model
APPLE SSD TS0128
$ cat /sys/bus/scsi/devices/0\:0\:0\:0/rev
0219
