You are booting off of a USB-connected drive. From your description, it sounds like the USB connection is not working correctly some of the time. Have you run the Verify Disk (not Verify Permissions) function in Disk Utility on the USB drive? Did it report any errors? It could be the USB hardware in the drive enclosure you are using is flaking out.
