I have a 5.1 setup in Ubuntu 10.04 LTS (I think), powered by ALSA
1.0.25 and pulseaudio 1:1.1-0.  (I got hooked when a friend lent me
his speakers, so I bought my own.)  When I run "speaker-test -c 6" (6
channels) it displays

 0 - Front Left
 4 - Center
 1 - Front Right
 3 - Rear Right
 2 - Rear Left
 5 - LFE

and with each line plays ~10 seconds of pink noise in the appropriate
speaker, so you can easily detect miswired or missing connections.  For
the first three (0 4 1) it's correct. #3 (Rear Right) plays from the rear
left, and the last two (2 5) don't play at all.  The audio goes from the
sound card to the subwoofer (3 stereo connections[1], all are seated on
both ends), and from there to the satellite speakers.  All connections
are color-coded so they're pretty hard to confuse.  The sliders and
switches in alsamixer seem to be correct, in that there is no slider
where one side is up and the other isn't.  I looked at
http://drona.csa.iisc.ernet.in/~uday/surround-pulse.shtml and all it said
to do either didn't apply (turning on switches that didn't exist) or was
done already.  Any ideas?

In alsamixer only the "Tone" switch is on (to make the bass/treble
sliders take effect); the rest are off.  They are:

Headphone LFE 1
Headphone Center 1
3D Control - Switch
Mic Boost (+20dB)
S/PDIF Optical Raw
Surround Phase Inversion
External Amplifier
SB Live Analog/Digital Output Jack
Sigmatel 4-Speaker Stereo
Sigmatel Output Bias

[1] The pairing is green=Front L&R, black=rear L&R, yellow=center&sub.
AIUI there's no standard for which is center and which is sub, but if
they're wrong I'd expect them to be reversed, not half-silent.

I remember trouble with ALSA a few years ago.  Most, if not ALL of those problems are gone with the latest Long Term Support (LTS) version and more so in 13.10.

The latest LTS is 12.10.  Have you considered upgrading and seeing if your issues go away?

Well now for some reason, LFE (sub) works.  The rear right still comes
out the rear left, and the rear left is absent.  The current .asoundrc is:

(plus some commented-out stuff)

Can you explain what the ttable columns mean?  The last number looks like
a level, and the second looks like an output channel.

Upgrading the distro is a last resort.  Many things break, including my
workflow.  IIRC ALSA is as modern as my distro allows.  Lemme check...
