Here's someone in 1998 discussing loop-and-a-half, and its clear from
context they consider it a well-known idiom

If Tim did invent it, he did so a long time ago, and it really caught on.

Of course, Tim's depth of knowledge of experience does suggest that he's
been programming since the time that punch cards were made of papyrus,
so its not out of the question.
