I use Calendar for this... add an alert to this item.  My OSX Calendar
is synced via iCloud with my iPhone.  So even if I am not near my
computer, I hear the alert from the iPhone.

You can also do repeating reminders in *drum roll* the Reminders app. : D

One easy possibility would be to set an "event" in the Calendar program 
and set it to repeat every day. You can then set a notification, which 
can include a sound, at the time of the event or just before. This kind 
of notification will certainly be noticed if you are in the same room 
with the computer. Others have mentioned "alarm" programs, which might 
give an even more noticeable sound.

OS X includes

	Reminders

	Calendar

Will both do it.  I'd tend to the former rather than the later for this 
purpose.  (I use it to make sure I take out the trash, recycling each 
week; pay various bills, cancel various contracts, ...)

In both cases, properly sync'd, the reminder will also sound on your 
iPhone, iPad, iPod, Watch if you're so equipped.

I had to do something similar, twice a day, for months.  I bought a 
small kitchen timer that worked at up to 99 hours. Once the alarm went 
off and was quieted, it was still set for 12 hours, rather than having 
it default to zero.  I used it very successfully.

Certainly a computer will work well for such things, but when I wasn't 
around the computer, or the computer was off, I found this way easier 
to deal with.

These days I might use the same thing but with my cell phone, which can 
make a ceaseless racket when it goes off.  Most of the computerly 
things, like Calendar, simply do a single distraction alarm and then 
quit. It's easily overlooked.

That's what a mobile device is for. Have it play an alert and/or vibrate
to get your attention. Anyway, I wasn't talking about missing an
on-screen alert fullstop. I was talking about missing it while looking
at the screen. If you've shifted alert dialog position away from the
centre of the screen, then yes - perhaps it is possible to miss them.
But I have iCal popping one up dead centre on my main screen - and that
is impossible to ignore _on my screen_ :-)

I also have Chrome (with and without systems with a Calendar app) set to
keep a pinned tab of Google Calendar open, so when an an alert happens
Chrome and that tab with it's alert are brought to the front. Between
that, iCal, and Calengoo's alert on my iOS device, I'm well covered.

Reminders seems to work OK, and I've got the warning synched and
repeated on my iPad.
However, my phone is not an iPhone so I can't synch it to the Mac.  Or
can I?

Not using Mac iCloud programs, no.  I don't trust iCloud for anything, 
so I don't use it.

Instead, I use a 3rd party app BusyCal which syncs to Google Calendar 
(and your local Apple Calendar but not to iCloud).

There may be Reminder apps for Android that sync to iCloud and the Apple 
ecosystem, but good luck finding them.

Your options are "get an iPhone" or switch to a more ubiquitous 
Android/Google solution on MacOS.  I use Wunderlist which can publish to 
Google Calendars but for simple reminders, just forget your phone or use 
a Pill Box timer.

With my mom, it was a constant fight to insist she take her meds on the 
recommended schedule.  She kept insisting she took them when I was there 
and know she didn't.  And as she's gotten older and the med schedule has 
gotten more complex, she clearly can't cope with it herself.  Short of a 
caregiver, we found something like this wasn't much help:
