I'm still not seeing what I expect.  Try these two images as a test:
misc/wap/APPLE-3-WAP-tel-06b.dsk
misc/wap/APPLE-3-WAP-tel-10b.dsk

And compare the hash of .RS232.  I think they should be the same, as 
they look suspiciously similar but for the locations of comment and dib. 
  Maybe you only removed the comment_start from md5 consideration?
