My model for usability always has been the "Global Link" from the TV
show "Earth Final Conflict".   There don't seem to be many pictures of
it available on the web ... best image I can find is here:

For those not familiar, the Global Link was a combination voice/video
phone and cloud enabled pad computer with a pull-out touchscreen that
(presumably) rolled into the body of the device when closed.  The
picture at the link above shows it partially extended or possibly
there were different models: I remember fully extended screens looking
to be roughly 6 inches wide.  
[Assuming 4 inch height that would make it ~7 inch diagonal.] 

A few years ago, a company called Polymer Vision created the "Readius"
- a 3G phone with a folding 5 inch E-Ink display.  It made a brief
splash in the news and then disappeared.  Probably not durable enough.
