Neither do I.  That's why I was asking wolfgang, but he doesn't know
either.

All I know is that it worked very well for Windows 98/SE for their DOS
console.  I don't know how much work they had to do.  It's possible
that in some cases they trapped instructions in V86 which switched to
PM32 and then they reissued in 16-bit RM.  I know that certain
instructions have to be emulated, and that there are 3 V86 modes since
it was extended twice:

I wanted to keep my OS independent of the BIOS.

V86 mode will switch back to protected mode for various reasons, but
mainly for interrupts.  That's probably the best control you can get
without an emulator.  You can always emulate instructions in PM that
break out of V86.  I haven't done anything with V86.  I have used
Win98/SE DOS console or other V86 mode monitors.  IIRC, Alexei Frounze
has used it.
