Boot from a floppy or USB stick.  Modify IVT.  Reboot hard disk.  Either
load HD bootsector to 7C00h and jump to it, or reboot via Int 19h.  On
modern BIOSes, Int 19h will continue along the boot chain to the next
bootable device.  This only works if the BIOS is set to boot from
multiple devices.  Also, if the BBS menu (BIOS Boot Specification) is
active (F11 or F12), then the user could boot another device of their
choice.  Some BIOSes can lock a machine to boot from only one device.
That would require a breach of the OS to install a malware bootloader.
Except, most BIOSes that I've seen cannot lock the BIOS so that it boots
only a single device.  There is usually a default list of a few
bootable devices which can't be disabled, but can be re-ordered.

A while ago, I attempted something similar before DOS booted.  I had
problems getting DOS to boot correctly after my change of an IVT
entry.  That was a trivial change to an unprotected OS on an
unprotected machine, so this isn't something which is easily
accomplished in my opinion.
