Because there are plenty of macros that cannot be expressed as inline
functions. A simple example:
#define MAX(x, y) ((x) > (y) ? (x) : (y))
An inline function would have to specify the type of the arguments.
(Of course the macro has the drawback that one of the arguments
will be evaluated twice.)

Once you've implemented while loops in the compiler, the effort to
implement for loops is minimal. It's not likely to take away any
significant amount of time that would otherwise be spent improving
optimization.
But even if it did, the point is to make it easier to write and
(more importantly) read C code. A typical C for loop:
for (int i = 0; i < MAX; i ++) { /* ... */ }
is easier to write and to read than the equivalent while loop,
where the initialization is above the top of the loop and the "i ++"
is buried at the bottom of the loop body. That expressiveness more
important than a little extra optimization.

Recursion is a useful tool, and not all recursion is tail-call
optimizable. You're suggesting a severe restriction on the
expressiveness of the language.
If you want a language in which you can't write a straightforward
binary tree traversal algorithm, I suggest calling it something
other than C, and not expecting many people to use it.

Presumably you'd be comparing it to C code written under the
same restrictions (no for loops, no non-tail recursion, etc.).
There's no reason a full C compiler couldn't optimize such code
just as well as a hypothetical subset-of-C compiler.
