Without VCPI enabled neither the DPMI host nor the DOS extender will have
a means to enter protected mode if something like JEMM386 or EMM386 is
running and providing EMS and/or UMBs.

If something like EMM386 or JEMM386 isn't running then the DOS extender
can just enter protected mode directly.  It doesn't need to use VCPI or
the already loaded DPMI host.

Looking at the source, that's in fact what DOS/32A will do.
