Probably the card.  It is a 128Gb SDXC that I paid $20 for, some Chinese knockoff probably.

My machine is a late 2012 MBP w/Retina and has been flawless since new.

Yeah I did that.  It needs the GUID partition table, won't work at all with Master Boot Record.
