What's the cycle time for the appearance of "unintended consequences"? A corporation or other large organizational entity spends months or years and megabucks planning a major change, then more months/years and more megabucks implementing it, then more time and money in shakedown and training. At some point the uninteded consequences begin to appear but probably aren't prioritized over getting the new $WHATEVER system working as planned. Eventually, the unintended consequences manifest as a sudden and shocking fuvg-storm or at least as a quiet but steady ingress of fuvg graually rising to knee depth in some departments.
