It was (and still is in some cases) a separate program.  It is "within
the compiler" only in the sense that pre-processing is a required part
of translating a source program.

It sounds like the only sane way to do it.

Do you mean you would simply never have a macro processor at the front
of a compiler?  That's a reasonable stance, but you might find you want
some mechanism to do the things one does in C using cpp.
