The documentation lists dozens of things that might be the cause, and 
cannot name the actual files, directories, acls, etc, that are involved.

Yes. I was there, coding them. Been a Unix aficionado since 1979, and 
coding since 1974. Still have millions of lines of my code running 
millions of machines worldwide.

The machines have moved on. Perhaps it's time we did also?
