The point is that the dispersion is not zero and the dispersion is in
two dimensions.  That means an inverse square law in the far field.

Laser diodes have very small apertures, hence large dispersion.

That would be true for the first few dozen meters.
After the beam has spread out due to diffraction effects, it will show inverse 
square behavior. 

You can reduce the spreading of the beam by collimating the  beam through a 
large telescope. After a few hundred kilometers or so, you'll be back to inverse
square.

I get it from measurements in our lab. Our laser is measured to have a spot 
diameter of 2.5 mm at 1 cm away from the collimating lens, and a diameter of 2.5 
mm about 1.5 meters away, with essentially the same brightness. That's VERY FAR 
from inverse square.

	Of course the whole point of a collimating lens is to turn the
	diverging light emitted from the fiber into a parallel beam.
	We have of course adjusted it to do that.

Of course at distances far enough away, its intensity does decrease as the 
inverse square (due to diffraction from the apertures). We cannot get nearly 
that far away in our lab.

Work it out. A typical laser will have an output beam diameter of 1.5 mm, a
"geometric" divergence due to the construction of the laser (one mirror, 
generally the output mirror, slightly concave), and a divergence due to
diffraction. The beam divergence for a typical laboratory laser is about 0.6
mrad which is approximately equally the result of diffraction and geometry.

The beam at 5 and 10 meters would be approximately 4.5 mm and 7.5 mm, which 
will not give anything even *close* to an inverse square square relationship.

You have to be "at distances far enough away" before you see an inverse square
relationship.

The equation for the beam diameter above is:
d ~= 1.5mm + 0.6E-3 *l  where l is the distance.

(7.5/4.5)/(10/5) = 0.83  (inverse square law = 1)

That does not change the fact that the relationship exists.

Maybe the misunderstanding arises because we have a different understanding
of what is an “inverse square law”.  To me it means that the equation
contains a factor of 1∕r² where r is the distance from the source.

That you cannot measure a thing does not mean that it is not there.  As you 
have said yourself, you can measure it if you are far enough away.

The collimating lens compresses the E-Strings that transmit the beam into a tight spot and that's why it does not obey the ISL. However at a far enough distance from the lens the beam will obey the ISL.

Right the parallel beam from the collimating lens  will diverge according to the ISL at a great distance.
