I have tracked the issue to this change:

The commit log message makes it seem that it is a refactoring change, 
but it inverts the sign on q->elevator test. It should have been 
documented as a bug fix and backported to stable branches.
The line was not further modified in the "master" branch.

I have rebuilt 4.19 from Debian testing with this change, and my system 
now works correctly.

In my opinion, it also relates to #913138 and maybe #904822.

Jens Axboe has determined that the proper fix is quite different:

This patch is already on its way to stable branches. I have tested it 
and confirmed that it resolves the problem.

(The line I originally found seems to actually be a mistake introduced 
in the master branch, but it somehow also prevents the deadlock).

I haven’t, but I’ve just run into this in a very uncomfortable
situation: I’ve just upgraded several virtualisation hosts from
jessie or stretch to buster, and one of them just started to
hang with exactly this problem. (They are all set up as LVM on
RAID, too, no cryptsetup.)

Ben et al. please do consider the fix for buster, otherwise
it’ll be unusable for this kind of setup.

Like you I'm looking forward for that kernel.
But, in the meantime, doesn't the workaround of disabling blk_mq work 
for you? See comment #15 and #20.
I used it for months without any problem.

I don’t know, because I have no means to trigger the bug. The three
other virtualisators I’ve upgraded at the same time did not trigger it.
My desktop machine also hits it only occasionally.

I’ve rebooted all four of them with the two kernel options, in the hopes
of staving off the bug, but in grub.cfg only, so they go away when the
fixed kernel will arrive.
