On Debian /usr is changed only when /var/lib/dpkg is changed (ideally).

The dpkg database IS read-only as long as you are not installing or 
removing packages, which you cannot do anyway if /usr is read-only.
In other words, the read-only state of the dpkg database is tightly 
coupled with the read-only state of /usr.

The only corner case which would require some more thinking is how to 
handle changing diversions of files in /etc in the systems with the 
read-only /usr, and for the time being "don't do that" could be 
a totally valid solution.

However, it does not cover packages that do not actually install
programs but only perform changes to /etc or install something to /opt,

While, AFAIK, it's indeed only Debian Policy stopping you from not
shipping /usr/share/doc/*/copyright, and that and common sense stopping
you from not shipping /usr/share/doc/*/changelog, that's just yet another
case of something broken that we probably are not required to support.
