Is it possible that the prefs file (or whatever file iTunes uses to
store that info) can't be written to?

Not here... OS X 10.9.2 / iTunes 11.1.5 (5)

Consistency from Apple apps should never be expected, only enjoyed when
it happens.

It's happened with me for at least the last three versions of iTunes
and the last three versions of OS X.

Very odd. Have you tried it with a new user account and/or a new iTunes
Library?

No, I haven't bothered. When I see the alert, I dismiss it and adjust
the volume.

Perhaps I missed it, but I don't recall seeing or reading about an alert
related to this issue. More details (screen shot) please?

Aha. I have never seen that message.

My first thought was perhaps you are accidentally pressing the volume
down key on your keyboard. Then I did a search and see that it may also
happen when you have certain Airplay speakers selected in iTunes:

<https://discussions.apple.com/thread/5434047>

Are either of those possibilities?

No, but it led me to the solution. I had only the iMac's speakers
selected. But when I clicked the "Multiple" button, it opened a
drop-down menu showing both the iMac's and the AirPlay speakers, each
with a volume control. The iMac's was turned all the way down. I
turned it up, then clicked on the "Single" button. The popup menu
closed, and now when I launch iTunes, the volume is set properly.

Interesting. I'm curious: have you *ever* use iTunes Airplay before?

Interesting, I don't think I've ever seen that.

What is your sound output set to?

I recommend leaving such images in place indefinitely, for the sake of
those who read these discussions later on. It may very well help someone
to see images in old Usenet posts. If you don't feel like storing them
in Dropbox, consider using one of the many free image hosting services
online.

I'd be happy with a "few days" so at least while the thread is
fresh(ish) anyone can see what it was about...

I would have liked to see it so that the occasionally reliable neurons
that I use for such might key on it some day in the future.

When I post such examples I usually leave them up at least for a few
days so that everyone with an interest can see it.

Not like my Dropbox use is anywhere even close to quota.

I point them to my own website, where I leave them indefinitely. Usenet
posts should live forever (or as long as possible), IMO.

Posts yes. "Media" I'm not all hard on about... links (websites) we
link to die off all the time... note a lot of people have -noarchive set.
