This is an issue with ALSA IMHO, it tend to get hiccups and you need to unmute 
master again.

Also KDE 3.5 is mainly unsupported nowadays, all the efforts been put into 
KDE4 and it starts to have most of the things KDE3.5 had, except some multi 
display supports (which has forced me to drop to a one monitor solution).

KMix kan be hooked to different volumes, say your master volume is muted and 
KMix is set to adjust PCM, then it may look like you have max volume and 
unmuted and still not a sound coming out.

Maybe you should change which source it should by default adjust?

Hmm, maybe I need to uninstall it? I can't remember if KDE relies on 
ALSA or if they are separated?

Kmix is set to Master for its "Master Channel". I tried telling it to 
use PCM, but that didn't work for volume controls, missing mute option, etc.

I guess I will have to use alsamixergui for now on for unmuting or use 
volume control. :(

If you don't like KDE4, you can use trinity. It's the continuation/fork
of KDE 3.5.

KDE 4.7 is much more complete and better than KDE 4.0, but, in my
experience with Fedora 15, I find it still too crashy (2 or 3
random crashes a day).

Your mileage may vary.
