Which countries? Certainly not in the EU:

Also, your addition "if you have a phone" falls short. At least it
should be "if you have a suitable smartphone". And even then I don't
see how it could be mandatory to:
- always carry the phone with you
- leave it on permanently
Even a "having a smartphone" requirement can be a serious showstopper.
According to the Dutch Bureau of Statistics, in 2019 (no later figures
available yet) 48% (!) of 75+ people didn't have a smartphone. And then
we're talking about the category most susceptible to the most serious
consequences fo covid-19.
