You missed the part where the calling party also sends an RCS saying I'm 
calling you, before being asked.

If this initial message is missing, then the called party sends an rcs 
back. Thus not always.

Just call using the exchange at the clinic. He can have a mobile phone 
with a number handled by the exchange, but I have seen this only on big 
places.
