Those input elements lack the name="..." attribute. By HTML specs, only
controls that have the name attribute can be "successful", i.e.
contribute to the form data set. So change e.g.
<input id="name" size="10">
to
<input id="name" name="name" size="10">

Many people have misunderstood statements about the name attribute being
deprecated. In HTML 4, it is deprecated for img and form elements. But
not for controls like input, where it constitutes an essential part of
functionality.
