ENDCASE performs a DROP and then resolves the branches. This leaves
the selector, 4, on the stack after the ENDCASE.

Back in the stone ages, the Forth Interest Group held a contest for a 
case statement. This version, contributed by a man named Eaker, won. 
Everyone ever since has hated the fact that ENDCASE does a drop (believe 
me, *everyone* makes this mistake at lease once), but we were all so 
burned by the gratuitous changes made by Forth83 (discussed in the "PICK 
changed..." thread) that no one has dared change it.

There was no 'standard' CASE before '94.  The TC could have rejected Eaker's
CASE on technical grounds (it has limitations beyond the built-in DROP) and
waited until something better came along.  Instead they went with the Eaker
CASE, enshrining it in stone and Forth for all time.  If anyone's to blame it's
the TC :)

Eaker's CASE was published in Forth Dimensions volume II number 2, the Case 
Contest issue. That was fourteen year earlier, in 1980.

Which no-one was compelled to use or implement until '94 standardized it.

Alternatives existed e.g. Baden's COND THENS primitives published long
before '94.  These were used in my own work with case statements:

Nor were they compelled to standardize it - particularly if it had issues that
you're still complaining about in 2014.

I don't have a problem with Forth Standards as I can ignore them when I find
something objectionable.  For those who have placed their trust and Forth's
reputation in Forth Standards, it may be a different story.

CASE looks nice at first glance, but there are some hidden gotchas.
I've always been uncomfortable with it.  It's evaluated serially by
definition, so it's less efficient than equivalents in other
languages.  (It's possible for an optimizer to determine that each key
is constant, unique and has no side effects and evaluate the statement
with a jump table, but that's a lot of work.  I don't know if any
system does it.)

There are many other ways to do multi-way branches in Forth.  However,
they tend to be rather specific to the problem in question: you might
have a simple range of 0...N, in which case a simple execution vector
is needed, or a lookup table if the keys are sparse, or maybe a couple
of conditional branches if there are few keys, etc, etc.

I'm surprised to find you espousing complexity before coming back to
the traditional Forth of simple specifics.

I last checked linear table search performance against the test/jump
pair performance 10-15 years ago on the then current x86
implementation. From memory, there was not a lot of difference.
Cache architectures are somewhat different now.

For application programmers, the Eaker CASE has the advantage of
producing simple, reliable and maintainable source code.

That is not true of any FORTH, being as it is, by prior art, an emulation
of the instruction set of the KDF9.

That it is not a HLL is demonstrated by the following ...

Suppose in C, that you transposed the brackets of an if statement ...

if ) x == 3 (

then it would not compile.

However, if in FORTH you were to code your verbs in the wrong order
then your program would still run, much as an assembler program will run
tho' the instructions were to be coded in the wrong order.

That is not to say that FORTH does not have its place in the world, for it 
does
in the two areas; either of bring a processor board up from scratch when the
development budget for a skunk-works project is not there; or in the case
of a TYRO, usually an uppity Hardy and not a Softy,  who is completely
ignorant of the processes of software engineering.

All mainstream real-time applications have moved away from assembler-like
systems except for the lowest-level board support packages.

FORTH, by its very nature as an emulation of a low-level stack-based
assembler instruction set simply cannot be use to produce "simple, reliable 
and maintainable source code"

Facts, not assumptions, You have conveniently excised the bit about reliable
systems, I see

You need to check the sources of your information, before resorting to

But, then again, in the wider scheme of software engineering, FORTH is but a

But forth is a nice interactive assembler for small flash systems like flashforth.

And I like it.

Could it be possible to insert a new word DEFAULT that does the DROP instead of ENDCASE ??

You could have

case  1 of 100 endof
      2 of 200 endof
      3 of 300 endof
      default 999 endof
endcase

without problem.  The endcase would still drop, but never be reached, 
because the DEFAULT is a catch-all statement.

I like that. I guess in this guise default is really a synonym for DUP:

: default compile dup ; immediate

Right?
