Bjarne Stroustrup has quite a lot of information, faqs, links, and 
papers on his webpage:

There is a section on "design and history", which is as definitive a 
reference as any.

(And note to Jerry - Stroustrup talks about the ARM as a step towards 
standardisation and a basis for a standard, but considers the ANSI/ISO 
standards to be the point at which the language actually has a proper 
standard.)
