One of the best jams I saw resulted in two consecutive cards being
torn lengthwise about half their length, and winding up overlapped.
(Try pressing your hands towards each other with fingers extended
to see what I mean.)
One jam left a card in contact with a rapidly spinning roller,
which burned a hole about 3/4 of an inch in diameter in the card.
We saw smoke coming out of the machine.

Yes, I've seen some nice (and amazingly neat) accordion pleats
put into cards.

One shop that went through a lot of cards bought them wherever they
could find them to save money. Most of these were overruns from
other applications, which is how I built such an interesting
collection of differently-printed cards. Sometimes the quality
of these cards was definitely dodgy. I suspect that the thickness
of some of them wasn't that tightly controlled, which resulted in
one operator turning the air blue as a result of all the jams.
When you fanned and joggled a fresh deck of cards before putting
them into the punch, you had to work a bit; the cards often stuck
together. I remember picking up a handful of cards and smacking
them against my other hand to break them apart. I almost broke
my hand; they were like a block of wood. Sometimes we'd stand
blocks of cards on edge on a machine or table top, where they'd
sit in a solid block with no rubber band holding them together.

Nice. My best card reader fix was when a timing belt broke.
The first tihng I did was go to the CE cabinet and get out the
manuals for the reader. I looked up the part number of the
broken belt, then called the service department. They were
used to service requests of the form "the machine doesn't work,"
so when I told them a timing belt had broken they started the
game of Twenty Questions to figure out which belt it was.
("What does it look like? Where in the machine is it?"
Et cetera.) I read off the part number and I could hear
the person on the other end light up. He quickly dispatched
a CE with the needed part.
Meanwhile, we had lots of work to do and didn't want to wait
until the CE arrived. So I took a lot of rubber bands and
wrapped them around the pulleys in place of the missing belt.
It took a couple of seconds for everything to spin up, but by
loading the program and getting it ready to start reading,
I was able to use the manual feed button to spin up the card
reader and feed the first card, then start the program before
the card reader motor timed out and shut down again. It worked
well enough for production to continue.

They must have been using optics on those 96 column cards, brushes wouldn't make contact through those small holes. The 80 column card had elongated holes for the brushes to work. I was referring to the statement that the 96-column card was introduced to keep costs down. as "less hardware was required..." I pointed out that the hardware required for the 80-column card was a cylinder and wire brushes. That's about as simple as you can get. and was used for some 40+ years. > The 80 column card had elongated holes for the brushes to work. Although I never saw any 90-column gear during my years with Burroughs/Unisys, even after the acquisition of Sperry/Univac.

I have some punched 90-column cards. I once tried to write a program to read them on an 80-column reader. The columns that were more or less aligned with the 80-column positions came out OK, but the ones that fell between the 80-column positions were, as I expected, unreadable. Too bad - I even had code working to translate the 90-column cards' arcane encoding, which followed no pattern that I could discern. The Univac card readers of the day derived column timing from a little drum attached to the feed rollers; one head wrote timing marks to the drum and and another head read them back after a suitable rotation of the drum to advance the card to the next column.
