Sure, but its very far from clear that even if he had fired it, and its far from clear that he actually was ever ordered to, that that would have seen a MAD utter insanity.

It is in fact much more likely that we will eventually know the detail of stuff like that claim about the russian sub captain that ignored the protocol to launch, whether that actually happened or not and whether that was in fact the protocol or not.

Also a great announcer of moonrise, from what I read. Is it still
in operation?.. The Flyingdales one is the UK was very funny, afaik,
it was there to warn the US of attacks from the East. There wasa story
that being in the way of the outgoing radio pulses was _very_ bad for health.
(All that besides, it seems that all that scheme was very innovative.)

Except that the transit time is a lot longer and there was a huge radar
system set up to give warning. It wouldn't have been _much_ warning,
about 15 minutes, but it wouildn't have been like you say.

If you're close enough to ground zero that the thermal shock ignites wood items, yes, duck and cover isn't a viable survival strategy. However, beyond the reach of the thermal shock, the shockwave will cause structural issues that duck-and-cover _will_ mitigate (falling ceiling tiles, lights, etc.). Much like an earthquake. So, while duck-and-cover is useless if you're within a mile or three of ground zero (depending on yield), it can be a valuable short-term survival strategy further out.
