"Security issues" != "attacks". They will always be a problem with closed source as well. Every week sees more Microsoft patches. Linux has been helped by the fact that it's less popular than Windows, and what there is of it is less monolithic, so it's not such an attractive target. And it is possible that (rightly or wrongly) the evil-doers view Linux users as more knowledgable than the average Windows jockey, and so less likely to engage in behavior (like running executable attachments from that nice fellow in Nigeria) that put themselves at risk.

Ontario health care costs are implemented like a US payroll tax. It is 2.5% (might be 2.75%) of gross earned income. This pays essentially all of the direct costs associated with an individual. Capital costs generally are not covered by the payroll tax. Family doctors in Ontario (differs by province) are private companies organized in one of three ways 1 Family doctors individual offices with one or two doctors and nurse and a office manager. Fee for service billing covers wages and office costs. Doctors double as business managers. 2) Walk in clinics with multiple doctors some specialists often associated with other specialized services in the same building, x-ray, MRI, CTscan and blood chemistry. Private companies that have fee for service billing. Management of the walk in clinics is done by professional administrators, the doctors do medicine. 3) In remote area's (like where I live) it is organized in a slightly different way. Family doctors are associated with a regional hospital and have clinics in several surrounding communities staffed with doctors and staff. The billing works the same but the Doctors are both employees and receive path of the billing. I have a family doctor that was paid an incentive to come here and she also doesn't have the responsibility to manage a business. It comes as a surprise to many that a great deal of the Ontario medical services is done by private companies. The biggest economy of scale is single payer bypassing the overhead of layers of insurance and billing overheads. Doctors costs are quite a bit lower, especially because malpractice insurance is much lower and frivolous law suits are a criminal offense in Canada. The overall cost is lower because a bigger amount of health services happen before a condition becomes chronic. People are encouraged to seek health care. Abuse yes and no. It happens but isn't a big issue. Billing is based on fee schedules and are generally generous Medicare which is a single payer system has similar cost numbers. Something that always comes up is rationing questions. I have never seen anything other than appropriate medical care both personally and with family and friends. The goals are effective treatment from appropriate providers and that seems to me to be the practice. w..

I, Osmium say that, based on personal experience. I live in one of the contiguous 48 states. I just reviewed my paper "health" file. Every single communication I have from Medicare says "for Part B". Much of the big money changing hands is, as we all know, in Part A. I really did break my arm I really did have skin cancer. I was also in the hospital for ulcers which I didn't mention. Again no billing info, I thought two samples were enough to mention. WRT your wife I don't know of a mechanism that would enroll her in Medicare in the circumstances you describe, are you positive it wasn't Medicaid?

Medicaid has needs testing and income limits. The laws around medicare are complex but there's pretty good info on the medicare and social security websites. Two things that kicked in Social Security Disability and later Medicare were blindness and kidney failure. She hit both of those in a short time due to side effects of diabetes. I've got her medicare ID card -- I think she had part B coverage and she was also on our private insurance as well from my work. I believe that the Kidney Failure started the the Medicare eligability... The medicare.gov website says: "I have ESRD. Am I eligible for Medicare? You can get Medicare no matter how old you are if your kidneys no longer work, you need regular dialysis or have had a kidney transplant, and one of these applies to you: You've worked the required amount of time under Social Security, the Railroad Retirement Board (RRB), or as a government employee. You’re already getting or are eligible for Social Security or Railroad Retirement benefits. You’re the spouse or dependent child of a person who meets either of the requirements listed above." It also says: "If you're under 65 and have a disability, you're automatically enrolled in Part A and Part B after you get Social Security or Railroad Retirement benefits for 24 months. Your Medicare card will be mailed to you about 3 months before your 25th month of disability benefits. If you don't want Part B, follow the instructions that come with the card. " So when she went totally blind over 6 months she may have qualified after 2 years of disablity... even before the kidneys failed and dialysis started. Medicare and the private insurance would flip between primary and secondary due to the amount billed or time of year... Wierd rules but both the insurance company and the rest of the medical system was aware of this. Medicaid does not come into play if your income was above the cutoff and I know my sysadmin job paid well above the Medicaid cutoff without even checking. Bill -- -- Digital had it then. I was asked to look at medicaid "up billing" and "fraud" estimated to be 25-30%. Federal pays 50% for state run operation. CMS (feds) were offerring to increase to 60% the fed. percentage funding for states that would pass legislation that would significantly cut fraud. Lots of states wouldn't pass the legislation because they were heavily lobbied by medicaid industry. State legislators got money from the industry for not passing the legislation ... they didn't get anything for passing the legislation ... even though it would save billions for states and the feds. the cut in overall billing would be more than the 20% increase that FEDs would pay (from 50% to 60%, actually reducing the total that FEDs paid) ... and the benefit to the state would be enormous 20% cut (from 50% to 40%) of a much smaller billing. Big win for everybody except for the legislators and the medicaid industry that are skimming tens of billions (and which totally swamps cases of individuals scamming the medicaid system). past posts: http://www.garlic.com/~lynn/2006o.html#61 Health Care http://www.garlic.com/~lynn/2006p.html#14 Health Care http://www.garlic.com/~lynn/2007k.html#19 Another "migration" from the mainframe http://www.garlic.com/~lynn/2007q.html#7 what does xp do when system is copying http://www.garlic.com/~lynn/2007t.html#34 Newsweek article--baby boomers and computers http://www.garlic.com/~lynn/2007t.html#35 Newsweek article--baby boomers and computers http://www.garlic.com/~lynn/2008i.html#98 dollar coins http://www.garlic.com/~lynn/2008n.html#17 Michigan industry http://www.garlic.com/~lynn/2009o.html#71 "Rat Your Boss" or "Rats to Riches," the New SEC http://www.garlic.com/~lynn/2010f.html#34 The 2010 Census http://www.garlic.com/~lynn/2010o.html#37 WHAT, WHY AND HOW - FRAUD, IMPACT OF AUDIT http://www.garlic.com/~lynn/2011i.html#12 Happy 100th Birthday, IBM! http://www.garlic.com/~lynn/2011i.html#31 Happy 100th Birthday, IBM! http://www.garlic.com/~lynn/2011i.html#37 Happy 100th Birthday, IBM! http://www.garlic.com/~lynn/2012.html#6 We are on the brink of a historic decision [referring to defence cuts] http://www.garlic.com/~lynn/2012h.html#69 Monopoly/ Cartons of Punch Cards http://www.garlic.com/~lynn/2012i.html#81 Should the IBM approach be given a chance to fix the health care system? http://www.garlic.com/~lynn/2012i.html#86 Should the IBM approach be given a chance to fix the health care system? http://www.garlic.com/~lynn/2012m.html#66 General Mills computer http://www.garlic.com/~lynn/2013e.html#89 What Makes an Architecture Bizarre? http://www.garlic.com/~lynn/2013l.html#64 OT: NYT article--the rich get richer http://www.garlic.com/~lynn/2014c.html#38 Royal Pardon For Turing http://www.garlic.com/~lynn/2014c.html#44 Royal Pardon For Turing http://www.garlic.com/~lynn/2014c.html#45 Royal Pardon For Turing http://www.garlic.com/~lynn/2014c.html#48 Royal Pardon For Turing http://www.garlic.com/~lynn/2014c.html#107 Royal Pardon For Turing http://www.garlic.com/~lynn/2014c.html#108 Royal Pardon For Turing http://www.garlic.com/~lynn/2014d.html#6 Royal Pardon For Turing http://www.garlic.com/~lynn/2015d.html#77 Western Union envisioned internet functionality http://www.garlic.com/~lynn/2015f.html#49 1973--TI 8 digit electric calculator--$99.95 -- virtualization experience starting Jan1968, online at home since Mar1970

It doesn't? I get wads of documentation ("THIS IS _NOT_ A BILL"),
pages of boilerplate along with a listing of each and every charge,
who it was by, for what service on what date, what was paid/denied/my
responsibility. And the admonition "If you notice something suspicious
that might be dishonest billing, you can report it by calling
1-800-MEDICARE, 24 hours a day, 7 days a week."
Maybe it's because I get Medicare through Part C (private insurance
add-on), but I've had several vendors for that and they all generated
similar reports, so I'd assume the substance of the reports is
mandated by Medicare.

I wasn't talking about that sort of fraud, I was talking about the inevitable higher costs involved with the advertising and churning of customers between different insurance companies that is unavoidable with any insurance system, which a universal single payer system doesn't have. > The childish Medicare system That is using insults instead of rational argument. > doesn't even tell patients what bills are being paid in their name. You are conflating single-payer with universal coverage; they are not the same thing. Obama knew he couldn't get single-payer through Congress; even the Dems wouldn't have supported it at the time. So, the compromise (Dole's proposal, later implemented by Romney in MA) was universal coverage within our existing multi-payer system. Once you have universal coverage, you can eliminate the exclusion for preexisting conditions. Single-payer is not required. On the other hand, assume one paid into the insurance system for their entire working career, is laid off, COBRA runs out and they're unable to get insurance because of an existing condition. Is that fair to the individual who paid so much into the system, to be denied just as the need arises? Of course, the individual mandate is intended to ensure that nobody waits until they get sick/injured before obtaining insurance.
