It does delete the trash and spam.

"Messages that have been in Spam more than 30 days will be automatically 
deleted."

"Messages that have been in Trash more than 30 days will be automatically 
deleted."

Trash is what you've deleted from the inbox, spam never got there. I think 
the inbox is forever or 15GB, whichever comes first. I probably don't have 
to worry about it but if I'm checking the spam trap I'll do some 
housekeeping.
