C was used on PDP-11s running Unix -- that's what it was designed for.
The early Unix linkers provided case-sensitive names distinct in the
first 7 characters.

No doubt C eventually ran on PDP-11s running other OSes.  Programs on
those systems would have to accommodate whatever restrictions the
tool-chain imposed.

Needing lower case is hardly a problem these days.

Yet it worked just fine -- go figure!

C was born on machines with operating systems that had both cases so C
source-code was never a problem but some systems did have limited
linkers.  The worst (of the first batch of C compilers) was on the
Honeywell 6000 whose linker used only one case (I don't recall which)
and had a maximum of 6 characters.  The IBM implementation also had only
one case for linked names but used the first 7 characters.

The advice was simple: for portability use lower case for external names
and keep them distinct in the first 6 characters.
