You are making unwarranted assumptions about the rules! Here is a GCD
algorithm as set of rules:
{<t: goto p>, <q: t = b>, <p: stop if b = 0>, <s: a = t>, <r: b := a%b>}
It is the same not mater what order the set is written and it will never
contain a duplicate because of what I've decided a rule is.
You may be right that the author made a mistake, but it's not certain
from what you said about it.

It is likely that the word is being used informally. Even before Church
and Turing et al I think the term "effective procedure" was used to
describe the notion, and it's important that it *wasn't* too formally
defined. If it is, phrases like "there is no effective procedure to
..." become statements about some particular formalism rather than
claims about anything that might reasonably be called an effective
procedure.

I think that's too loose because so many C functions are formally
undefined for some or all inputs.
