Thanks. It's an old version, I think from the package that included PS 
3. So maybe ten years ago?

In any event, I'd really like to get the form working properly- as it 
did when I first filled it out two years ago. It was problematical last 
year, but somehow it came around. Now it won't. I think I will try as a 
different user.
